🏠 Residential Habilitation & Support (RHS)

At 4 Walls For All, every person deserves the opportunity to thrive in a safe, supportive, and familiar environment. Our Residential Habilitation & Support (RHS) services help to empower individuals with intellectual and developmental disabilities (IDD) to live as independently as possible—right in their own homes or family settings—surrounded by care, consistency, and dignity.

RHS is a person-centered, goal-oriented service provided under the Community Integration and Habilitation (CIH) Waiver. It is customized to fit each individual's Person-Centered Individualized Support Plan (PCISP). The program emphasizes developing or maintaining the skills needed for meaningful and successful community living.

Our services are flexible and uniquely designed to support each individual's strengths and needs. We assist with:

• Adaptive Skill Development: Helping individuals learn or enhance skills like cooking, cleaning, budgeting, grooming, and personal safety. 
• Activities of Daily Living (ADLs) include personal care, bathing, dressing, eating, mobility support, and medication assistance. 
• Community Inclusion: Supporting individuals to participate in local events, spiritual gatherings, community activities, or volunteer opportunities. 
• Transportation Ensuring access to appointments, shopping, employment, recreational activities, and more. 
• Educational & Leisure Support Engaging individuals in hobbies, adult learning, creative arts, and structured leisure to promote joy and growth. 
• Health & Wellness Support: Monitoring health routines, supporting diet and exercise plans, and assisting with medical coordination. 

Services can be scheduled for a few hours per week or up to  twenty-four hours a day, seven days a week  (24/7), depending on the individual's needs and authorized waiver funding. Two service levels are available:

• RH10 – Less than 35 hours/week 
• RH20 – 35+ hours/week 

Residential services are delivered wherever the individual calls home, including:

• Their apartment or home
• The family home
• Approved community environments 

In some cases, a legal guardian or family member may be approved to serve as a paid caregiver if:

• The individual is 18 or older
• The caregiver does not exceed 40 paid hours/week
• All provider and waiver guidelines are followed
