4 Walls For All adopts a unique approach to promoting meaningful growth and development both at home and in the community. Our Individual Day Habilitation services are specifically designed to support adults with intellectual and developmental disabilities, focusing on helping them acquire practical life skills, develop social abilities, and engage in enriching experiences within the community.

🌟 What Is Individual Day Habilitation? 

Individual Day Habilitation is a personalized service that is not one-size-fits-all. It is designed under the Community Integration and Habilitation (CIH) Waiver and focuses on promoting independence and improving daily living skills. This is achieved by working towards the goals outlined in each Person-Centered Individualized Support Plan (PCISP). Services are provided in community-based settings, rather than in residential facilities, and can complement therapy services when appropriate.

Our services are carefully designed to address each individual’s unique needs and goals through personalized, one-on-one support. We assist with developing everyday skills, including grooming, hygiene, cooking, and managing basic household responsibilities. Our comprehensive approach also prioritizes personal safety and health by supporting healthy routines, helping individuals understand their medications, and guiding them to navigate public spaces safely. We encourage and foster communication by assisting participants to develop their interpersonal and expressive skills. Additionally, we foster confidence for community engagement, enabling participation in local activities that range from shopping trips to public events. To promote enjoyment and recreation, we create opportunities for leisure through the arts, music, physical activities, and nature outings. Above all, we are committed to empowering participants to make informed choices and develop self-direction, supporting them in shaping their own lives.

🫳  Who Can Participate? 

Adults aged 18 and older who qualify for services under the CIH Waiver are eligible for these services. Our person-centered approach ensures collaboration with families, support teams, and clinicians to align with the individual's goals and preferences.
