🧑‍🦯 Participant Assistance Care (PAC)

Supporting Independence, One Task at a Time

At 4Walls For All, we believe that every individual deserves the opportunity to live with dignity and independence. Our Participant Assistance & Care (PAC) services are designed to provide essential daily support to individuals with intellectual and developmental disabilities, helping them manage tasks at home and engage in their community with confidence.


🌟 What Is Participant Assistance & Care?
PAC is a flexible, direct support service offered through the Family Support (FS) Waiver. It provides hands-on help and oversight to support individuals in maintaining their routines, remaining safely in their homes, and staying involved in their community. All support is customized according to the individual’s Person-Centered Individualized Support Plan (PCISP).


🧩 What Does PAC Cover?
PAC services can vary based on individual need, and may include:

  • Personal Hygiene Support – Assistance with bathing, grooming, dressing, and toileting. 
  • Meal Help – Supporting meal preparation and nutritional needs. 
  • Errands & Shopping – Help picking up groceries, prescriptions, and more. 
  • Appointments – Support in scheduling, attending, and navigating medical or personal appointments.
  • Household Tasks – Light home upkeep like laundry, tidying, and organizing.
  • Paperwork Assistance – Help understanding mail, managing bills, or completing forms.
     
  • Mobility Aid – Physical support with movement, transfers, or equipment use. 
  • Behavior Plan Support – Reinforcing behavioral strategies and safety protocols. 
  • Daily Supervision – Ongoing oversight to ensure safety and consistency throughout the day.
     

👥 Who Can Deliver PAC Services?
Services may be provided by trained Direct Support Professionals or, in approved cases, by a family member or guardian—if the individual is 18 or older. This approach allows for familiar, trusted care with proper structure and accountability.


🏠 Where Services Take Place
PAC support is delivered wherever the individual needs it most:

  • At home
  • At a family member’s residence
  • Out in the community (during errands, appointments, etc.)
